Proof of medical exemption can be submitted directly to Med+Proctor beginning in July 2024. Medical exemption requires a letter from a health care provider stating that the immunization is medically contraindicated. With proper documentation, USF will accept medical and/or religious exemptions to the COVID-19 vaccine requirement. If you have received one or both doses of the COVID-19 vaccine plus boosters, please submit the results through Med+Proctor beginning in July 2024. USF has partnered with Med+Proctor to track proof of vaccination. Click here for COVID related resources and information. This includes new students arriving on campus next fall. USF requires all students, faculty, and staff be fully vaccinated with boosters if eligible.
